Plan Evaluation Assessment

Our Plan Evaluation Assessment is delivered over 3–6 weeks and is designed to help:

  • People with an existing NDIS plan who need support to implement their plan or prepare for a review or appeal

  • Individuals seeking NDIS access, including those who have received a rejection

  • Service providers or organisations funding an evaluation for participants with complex needs, service barriers, or funding shortfalls (with written consent)

What You’ll Need

  • NDIS Plan or NDIS Access Document, plus

  • Up to two optional supporting documents, such as diagnosis letters, allied health reports, behaviour support plans, medical or carer impact statements

Additional documents may be requested. The minimum requirement is a NDIS Plan or your completed Access Document.

How It Works

  • Intake form, document review, and an initial one‑hour consultation

  • Report preparation over 3–6 weeks, depending on complexity

  • Follow‑up consultation to discuss findings and next steps

  • Finalised report with clear suggestions, support options, and action steps

  • Final session to review and refine the report
