In Depth Plan Evaluation
Assessment outline -
Complete our intake form, including submitting evidence documents to be reviewed and schedule an initial one-hour session with our consultant.
Our consultant will prepare a report or action plan based on your discussion. This can take anywhere between 3 to 6 weeks, depending on the amount of evidence you have provided/complexity of the situation. We may contact you with questions or for further clarification while the report is being prepared.
The next one-hour session with our consultant will be scheduled. You will discuss further details and create a plan/timeline of suggested actions moving forward.
Our consultant will prepare the report of suggested actions and recommended supports and services, including guides for your future reference.
Schedule a final one hour-session where you will discuss the report and our consultants recommendations, ask any final questions and any additional amendments will be made to the report.
Receive the completed in-depth plan evaluation report.
Assessment outcome
A written report, including lists of suggested NDIS supports and services, community and mainstream options and recommended organisational contacts.
An action plan collated into an easily accessible timeline based on the evidence provided by you.
Your own NDIS Reference Folder to assist organisation of documents, evidence and NDIS files.
Handouts, guides and printable documents to help support your NDIS journey to be used for the future.
A Plan Evaluation Assessment is completed over a three to six week period. This assessment can be suitable for
A participant with an existing NDIS plan wanting an evaluation. This may be to overcome challenges in implementing a plan, to prepare for a plan review or appeal, or after receiving an impairment notice.
A person seeking access to NDIS who wants to be well prepared, or has received a rejection in accessing the NDIS and wants to explore their options.
A service provider or organisation funding an evaluation for a participant who consents to a Plan Evaluation Assessment due to complex needs, service delivery barriers or funding shortfalls.
Evidence Documents to be provided
When completing our intake form for this assessment you will be required to provide us with the following documentation for review:
NDIS Plan or NDIS Access Document, and
a maximum of two documents from the following list (optional)
Original Professional Diagnosis Letter
Occupational Therapist Functional Capacity Assessment
Behaviour Support Plan - if you have a Behaviour Practitioner
Allied Health Reports from services such as Speech Pathologists, Psychologists, Physiotherapists
NDIS Decision Letters
Medical Letters
Medical Plans - such as Epilepsy Management Plans
Carer Impact Statements
Additional documents that may be helpful.
Throughout the assessment process our consultant may request additional documents to assist with the evaluation if they are available. If you do not have any of these listed documents, the minimum requirement for this assessment is an NDIS Plan or NDIS Access Document.
