Plan Evaluation Assessment

Our Comprehensive Plan Evaluation Assessment is delivered over 3–6 weeks and is designed to support:

  • People with an existing NDIS plan needing help with implementation, review, or appeal

  • Individuals seeking NDIS access, including those who have been rejected

  • Service providers or organisations funding evaluations for participants with complex needs, service barriers, or funding shortfalls (with written consent)

What You’ll Need to Get Started

  • Your NDIS Plan or NDIS Access Document

  • Two optional supporting documents, such as:

    • Diagnosis letters

    • Allied health reports

    • Behaviour support plans

    • Medical or carer impact statements

How It Works

  1. Book Your Appointment
    Choose a suitable online booking time for your initial consultation.

  2. Upload Your Documents
    Submit your NDIS plan/access document and any supporting information.

  3. Initial Consultation (1 Hour)
    We discuss your goals, review your documents, and identify any gaps.

  4. Progress Check-In (1 Hour)
    We discuss your draft report, gather your feedback, and confirm any final evidence needed, and answer your questions.

  5. Final Meeting & Report Delivery (1 Hour)
    You receive;

    • a Comprehensive Plan Evaluation Report including a gaps analysis and list of services and support options

    • a personalised evidence folder guide,

    • a clear step by step plan with the next steps,

    • access to Proving the Complex Video E-course,

    • educational and resource documents, such as Kleoss’s Document Check Up List, and much more.

    From our first meetings, to delivering the final report, the process occurs over a 3 to 6 week period.
